“+1 to Skill” means you get one extra Skill point to spend in that Skill tree as long as you have specialized that Skill. 

“+1 Max” applies to Skills that summon something, like Companions or Forge Weapons, by giving you more of said thing equal to the number on the modifier. For example: +3 Maximum Forged Weapons.
